| - With repeated FDG-PET/CT examinations carried out before, 14 days after the beginning and 4 weeks after the completion of neoadjuvant chemotherapy for adenocarcinoma of the esophagus or esophago-gastric junction, we will examine how correlates the decrease of tumor metabolic activity, expressed as a decrease of consumption of 18-FDG in tumor tissue (rated according to criteria PERCIST 1.0) early (14 days) after the start of neoadjuvant therapy with the degree of histopathologic tumor regression on the definitive histopathologic examination after operation that will follow the completion of neoadjuvant therapy. In the case of confirmation of the correlation, we set the threshold of decline of the tumor metabolic activity, through which we can distinguish with reasonable accuracy patients who will respond well to preoperative chemotherapy from patients resistant to these therapy early in the course of the treatment. - Pomocí opakovaného FDG-PET/CT vyšetření, provedeného před, 14 dnů po začátku a 4 týdny po ukončení neoadjuvantní chemoterapie u adenokarcinomu jícnu nebo EGJ budeme zjišťovat, jak koreluje pokles metabolické aktivity tumoru vyjádřený poklesem konzupce 18-FDG v nádorové tkáni (hodnoceným podle PERCIST 1.0 kriterií) včasně (14 dnů) po začátku neoadjuvantní léčby s mírou histopatologické regrese tumoru v definitivním histologickém resekčním preparátu po operaci, která bude následovat po ukončení neoadjuvantní léčby. V případě potvrzení předpokládané korelace stanovíme prahovou hodnotu poklesu metabolické aktivity tumoru, pomocí které s dostatečnou spolehlivostí můžeme včasně odlišit pacienty, kteří budou dobře reagovat na předoperační CHT od pacientů rezistentních na tuto léčbu.
