| Description
| - The Ig-specific single-cell RT-PCR in B cells of patients with CVID, IgAD/IgMD will be used to analyze somatic hypermutation, affinity maturation/isotype switch, proliferation and activation, BCR signalling and differentiation stadium of B cell subsets before and after stimulation with IL-4, IL-21 and anti-CD40 together with tetanus toxoid/pneumococcal saccharide. The bank of EBV-transformed B-lymphocyte lines will be generated to address their contribution to the pathogenesis of mentioned diseases. The publication in peer-reviewed scientific journals, extension of knowledge on pathogenesis of these diseases, determination of differentiation stadium and BCR signalling in B cell subsets and new classification diagnostic criteria represent anticipated outputs. The improvement of diagnosis/ therapy leading to the selection of particular individual immunotherapy is the main focus of the study in accordance the priorites of declared „Programme of Research and Development of the Czech Ministry of Health“. - U pacientů s CVID, IgAD a IgMD budeme s využitím Ig-specifického RT-PCR na úrovni jedné buňky analyzovat somatické hypermutace, známky afinitní maturace, a izotypového přesmyku, schopnost proliferace a aktivace, stav BCR signalizace a diferenciační stadium v jednotlivých B lymfocytárních subpopulací před a po stimulaci IL-4, IL-21 a anti-CD40 v kombinaci s tetanovým toxoidem a pnemokokovým sacharidem. Bude vytvořena banka EBV-transformovaných B-lymfocytárních lini s cílem stanovit podíl B lymfocytů v patogenezi uvedených chorob. Očekávanými výstupy projektu jsou publikace v impaktovaném zahraničním vědeckém časopise, doplnění znalosti etiopatogeneze CVID a IgAD/IgMD, definování diferenciačního stadia studovaných subpopulací a kvality BCR signalizace, definice nových klasifikačních kritérií s cílem zlepšení diagnostiky a terapie ve smyslu nastavení konkrétní individuální imunoterapie pacientů (v souladu s prioritami Resortního programu výzkumu a vývoje MZ ČR).
