Description
| - Capillary leak syndrome (CLS) is characterized by an increase in vascular permeability accompained by extravasation of fluids and proteins from the capillary vessels into the tissues resulting in interstitial edema, decrease in microcirculatory pefusion and different types and severities of organ damage. CLS was observed in various pathological conditions- it occurs in patiens with sepsis, burns, trauma, surgery and other disease, in cancer patients treated with interleukon -2,-3-4, immunotoxins, GM-CSF, and cytostatics. Recently we have found that CLS in 37% of neonates critically ill treated with gentamicin (Ge) during the first postnatal week, CLS makes kinetically guided Ge dosage prediction impossible. It may be due to significant changes in the PK of Ge that is distributed to extravascular fluid and eliminated via glomerular filtration, and whose efficacy, moreover, is more dependent on plasma concentration than on the dosing. Volume of distribution was increased in experimental rats compared with controls, but the elimination halftime was shorter in LPSIL-2 rats and clearence per kg was increasedin LPSIL-2 animals. (en)
