The most frequent causes of accidental exposures were inattention, inadequate supervision, non-standard procedures, stressful situations, and also inadequate communication, reading of documentation, and knowledge or experience. The most frequent mistakes were patient exchange, wrong plan, incomplete irradiation, incorrect patient positioning, and incorrect irradiator or table parameters.
