V prvn\u00ED kazuistice popisujeme p\u0159\u00EDpad 53let\u00E9 \u017Eeny, u kter\u00E9 bylo p\u016Fvodn\u011B pro posti\u017Een\u00ED kr\u00E1tkodob\u00E9 pam\u011Bti vysloveno podez\u0159en\u00ED na Alzheimerovu nemoc. V n\u00E1vaznosti na akutn\u00ED zhor\u0161en\u00ED stavu s poruchou v\u011Bdom\u00ED a epileptick\u00FDmi z\u00E1chvaty byla u pacientky na z\u00E1klad\u011B zm\u011Bn sign\u00E1lu MR mozku ve sp\u00E1nkov\u00FDch laloc\u00EDch stanovena diagn\u00F3za limbick\u00E9 encefalitidy. U druh\u00E9ho p\u0159\u00EDpadu, 38let\u00E9ho mu\u017Ee, se onemocn\u011Bn\u00ED manifestovalo poruchami pam\u011Bti a chov\u00E1n\u00ED s pom\u011Brn\u011B rychl\u00FDm rozvojem, co\u017E budilo podez\u0159en\u00ED na psych\u00F3zu. N\u00E1sledn\u00FD rozvoj epileptick\u00FDch z\u00E1chvat\u016F, intenzitn\u00EDch MR zm\u011Bn v medi\u00E1ln\u00ED \u010D\u00E1sti tempor\u00E1ln\u00EDch lalok\u016F a pozitivita autoprotil\u00E1tek proti nap\u011B\u0165ov\u011B \u0159\u00EDzen\u00FDm drasl\u00EDkov\u00FDm kan\u00E1l\u016Fm potvrdila diagn\u00F3zu limbick\u00E9 encefalitidy. "Limbic encephalitis was initially described as a rare paraneoplastic syndrome characterized by rapid progression, anterograde amnesia, mood disturbances, psychiatric symptoms and temporal lobe epilepsy. In the last decade, the concept of limbic encephalitis has expanded \u2013 apart from any paraneoplastic aetiology, it may occur as an infectious or autoimmune disorder (that may or may not be associated with antineuronal antibodies). Due to its various non-specific symptoms, early diagnosis of limbic encephalitis is frequently difficult. The first reported case, a 53-year-old female, presented with short-term memory loss, and was treated initially for Alzheimer\u2019s disease, later developing typical symptoms of limbic encephalitis with rapid alteration of consciousness, accompanied by seizures and discrete temporal lobe signal change on MRI. The second case was a 38-year-old male with rapidly progressing amnesia and behavioural disturbances. Development of epileptic seizures, typical medial temporal lobe signal change on MRI and abnormal increase of antibodies against voltage-gated potassium channels confirmed a diagnosis of limbic encephalitis. Both cases improved after corticosteroid therapy. No paraneoplastic origin was found in either case. We discuss the most usual signs and symptoms of limbic encephalitis, focusing on recent classification, diagnostics and treatment of this often under-diagnosed disorder."@en . .
