Androgenetic alopecia (AGA) is the most common form of alopecia affecting millions of men and women. AGA is an androgen hormone dependent process involving continuous miniaturization of sensitive hair follicles in both men and women, who are genetically predisposed. Hair loss in women is usually much less severe than in men, newertheless in affected women AGA causes serious psychological problems and its etiopathogenesis, especially in women, is not yet fully understood. However, experimental and clinical advances elucidate some pathogenetic steps leading to AGA. Currently most promissing approaches to the treatment of baldness in men are drug therapies, such as topical minoxidil and finasteride administered systemically. AGA in women can be treated with topical minoxidil, too, but in contrast to men antiandrogens administered systemically are frequently used. Androgenetická alopecie (AGA) je nejčastější formou alopecie, postihující milióny mužů a žen. AGA je proces, závislý na androgenních hormonech, mající za následek kontinuální miniaturizaci citlivých vlasových folikulů u geneticky predisponovaných mužů i žen. U obou pohlaví se předpokládá polygenní způsob dědičnosti, zahrnující několik genů. Řídnutí vlasů u žen je obvykle méně výrazné, než u mužů, nicméně u postižených žen způsobuje závažné psychické problémy. Etiopatogenze AGA, zejména u žen, není dosud plně objasněna. Pokroky v experimentální a klinické medicíně však postupně objasňují patogenetické stupně, vedoucí k AGA. V současnosti je u mužů nejslibnější zevní léčba minoxidilem a systémová léčba finasteridem. AGA u žen může být rovněž léčena minoxidilem, ale na rozdíl od mužů zejména antiandrogeny podávanými systémově.
