Major surgery, polytrauma and burn are often accompanied by a masive activation of the immune system. Due to counterregulatory mechanisms the hyperinflammation is followed by a temporary immunodeficiency that may lead to immunoparalysis and multiple organe failure. This postinflammatory immunodeficiency is characterized by altered function of monocytes. These cells seem to be impaired in antigen presentation and T lymphocyte stimulation. One potential mechanism for macrophage activation is through engagement of costimulatory molecules, in particular CD86. In this study we determined HLA-DR and CD86 expression in monocytes in critically ill patiens and helathy controls. The diminished expression and low mean fluorescence density HLA-DR on monocytes of critically ill patiens confirmed the down regulation of monocytes functions and has been proposed a realiable marker of immunosuppression...
