Úvod: Antipsychotika 2. generace nemají typické nežádoucí účinky konvenčních antipsychotik. Metodika: 34-letá pacientka s periodickou depresivní poruchou (dle MKN-10) a opakovaným sebepoškozením byla léčena lithiem, clonazepamem a ziprasidonem. Výsledky: Devátý den léčby ziprasidonem se objevila galaktorhea. Ta trvala do 36. dne léčby ziprasidonem. Hladina prolaktinu byla 28 ng/ml. Hormóny štítné žlázy (TSH, T3,T4) a plasmatická hladina lithia byly po dobu léčby ziprasidonem v normálním rozmezí. Galaktorhea zmizela dva týdny po vysazení ziprasidonu a hladina prolaktinu poklesla na 18 ng/ml. Závěr: Psychiatři si musí být vědomí skutečnosti, že i antipsychotika 2. generace jako ziprasidon, mají potenciál vyvolávat vedlejší účinky vyplývající z blokády dopaminových D2 receptorů, jako je například galaktorhea. Objectives: Second-generation antipsychotics presumably lack the typical side effects of conventional antipsychotics. Methodology: A 34 year old Caucasian woman with ICD-10 diagnosis of Recurrent depressive disorder with current moderate symptoms, and with a history of repeated self-injury was treated with lithium, clonazepam and ziprasidone. The plasma levels of prolactin were determined by the Microparticle Enzyme Immunoassay on 14th day of ziprasidone treatment and two weeks after the ziprasidone withdrawal. Results: On the ninth day of ziprasidone administration, galactorrhea appeared. After 36 days of ziprasidone therapy, galactorrhea persisted. The prolactin plasma level was 28 ng/ml. Thyroid tests and the lithium plasma level were within the normal range during ziprasidone treatment. Two weeks after the ziprasidone withdrawal, galactorrhea disappeared and the prolactin level decreased down to 18 ng/ml.
