| - Cíle: Indikace pro elektivní krční disekci u N0 pacientů s karcinomem příušní žlázy jsou nejednotná. Proto byla u těchto tumorů testována možnost provedení a účinnost radionavigované biopsie sentinelových uzlin. Metody: U 7 N0 pacientů s karcinomem příušní žlázy byla provedena předoperačně lymphoscintiografie následovaná parotidektomií a elektivní krční direkcí s radionavigovanou identifikací sentinelových uzlin. Histopatologický stav jednotlivých sentinelových uzlin byl pak porovnán, sestaven z ostatních uzlin. Výsledky: Biopsie sentinelových uzlin přesně odrážela pozitivní a negativní histopatologický stav ostatních uzlin u 3, resp. 4 pacientů. Nebyl žádný případ falešne negativity sentinelové uzliny. Závěry: Studie naznačuje, že biopsie sentinelových uzlin by mohla poskytnout spolehlivé informace o stavu regionálních lymfatid, což by umožnilo indikovat krční disekce na základě skutečné přítomnosti mikrometastáz. Než však bude tato metoda zavedena do běžné praxe, bude zapotřebí rozsáhlá multi (cs)
- Objektive: Indications for elective neck dissection in N0 staged patients with parotid gland carcinomas are inconsistent. A pilot study was perfored to test the feasibility and efficacy of radioguided sentinel lymph node biopsy (SLNB) in these tumors. Methods: In seven consecutive N0 patients with parotid gland carcinoma preoperative lymphoscintigraphy, followed by parotidectomy and elective neck dissection with radio-guided identification of sentinel lymph nodes (SLNs) was performed. The histopathological status of particular SLNs was then compared to that of the remaining lymphatic basin. Results: The SLBN exactly reflected the positive and negative histopathological status ot the remaining lymphatic basin in three and four patients, respectively. There was no case of false negativity of the SLN. Conclusions: The study suggests that SLNB could provide reliable information on the status of regional lymph nodes, making it possible to base the neck dissection on the actual presence of the micrometas
