| - This study described the first molecular characterisation of clinical isolates of vancomycin-resistant enterococci (VRE) in the Czech Republic. Of 2647 patients isolates of Enterococcus spp. from 1997-2002, 121 (4,6 %) were identified as VRE. The most common isolates were VanA Enterococcus faecium (78 %) and VanB Enterococcus faecalis (10 %). In addition, five VanA E. faecium isolates were obtained from environmental and staff sampling. Macrorestriction analysis of Smal restriction fragment length polymorphism was performed for 54 VanA E. faecium clinical isolates and the five VanA E. faecium envirnomental isolates. Thirty-two unique restriction endonuclease patterns were identified, including two predominant clonal types represented by five or more isolates. Two environmental VanA E. faecium isolates were closely related to two patient isolates, which had an identical Smal macrorestriction pattern. - Cílem uvedené práce bylo stanovení genetické příbuznosti kmenů Enterococcus faecium VanA izolovaných z klinického materiálu pacientů Hemato-onkologické kliniky Fakultní nemocnice Olomouc v období 1997-2002 a z prostředí uvedené kliniky, včetně biologických materiálů od ošetřujícího personálu a na základě výsledků pak formulovat hypotézu o zdroji a šíření VRE. Celkem bylo ve sledovaném období izolováno 2647 kmenů Enterococcus sp., přičemž 121 kmenů (4,6 %) bylo identifikováno jako VRE. Nejčastěji se jednalo o kmeny E. faecium Van A (78 %) a E. faecaůis VanB (10 %)- Z prostředí uvedené kliniky, včetně biologického materiálu ošetřujícího personálu, bylo izolováno 5 vankomycin-rezistentních kmenů E. faecium VanA. U 59 kmenů bylo identifikováno 32 odlišných profilů, jejichž podobnost se pohybovala od 35 do 98 %. Mezi těmito profily byly dále identifikovány 2 často zastoupené klonální typy.
