Description
  • Varicella zoster virus typically causes benign disease in childhood - chickenpox - and can reactivate in adults as herpes zoster (HZ). However in immunocompromised patients, the infection with VZV can lead to severe complications leading to a fatal outcome. Here we describe a patient, who acquired primary VZV infection during 3 weeks lasting corticosteroid treatment. The disease took a fulminant course leading to a liver failure and severe coagulopathy resulting in death after 9 days post admission to the hospital, despite an intensive antiviral and supportive treatment. Wild-type VZV DNA was detected from multiple samples from esophagus, liver and skin. Genotypic analysis based on characterization of single nucleotide polymorphisms in open reading frames (ORFs) 54, 38 and 62 together with genotyping of ORFs 22, 21 and 50 characterized this VZV isolate as M2 strain, which is typically found in tropical countries. This is the first description of such strain from a patient in Czech Republic.
