  • Aim Small-fiber neuropathy (SFN) is reported to be associated with a range of diseases or conditions that are partly different from those associated with large-fiber affection. The aim of the study is to evaluate aetiology in a group of SFN patients in comparison with mixed (i.e., small- and large-) fiber neuropathy (MFN). Methods A detailed medical history was taken and exhaustive blood tests, chest radiography and lumbar puncture were performed in a group of 84 patients with “burning feet” syndrome and clearly proven affection of small nerve fibers (verified by abnormal thermal thresholds and reduced intra-epidermal nerve fiber density in the distal calf). Thirty-three of them also exhibited nerve conduction abnormalities as a sign of large-fibre involvement (MFN group). Results The range and proportions of causes were similar in SFN and MFN subgroups.   • Cíl. Neuropatie tenkých vláken (SFN) je dle literatury asociována s širokým spektrem nemocí a stavů, které se zčásti liší od těch asociovaných s postiženíh silných vláken. Cílem bylo zjistit etiologii ve skupině pacientů s postižením typu SFN ve srovnání se skupinou s postižením jak tenkých, tak silných vláken - smíšený typ neuropatie (MFN). Methoda. Byla odebrána detailní anamnéza a provedeny rozsáhlé serologické testy, RTG hrudníku a lumbální punkce byly provedeny u skupině 84 nemocných se syndromem pálivé bolesti v nohou "burning feet" a jasně prokázanou afekcí tenkých vláken verifikovanou pomocí abnormálního termického prahu a sníženou intraepidermální hustotou v distální části bérce. U 33 pacientů byly navíc přítomny abnormality kondukčních studií jako známka současného postižení silných vláken (skupina smíšené neuropatie - MFN). Výsledky. Spektrum a podíl příčin byly podobné ve skupině SFN a MFN. Diabetes mellitus a chronický abusus alkoholu byly nejčastějšími příčinami (33%; 15%), zat (cs)
