  • An overview of current literature on deep sternal wound infection (DSWI) following cardiothoracic surgery, its epidemiology, risk factors, and treatment strategies development. The chapter will comprehensively discuss the current options of preventive measures which reduce the risk of DSWI, current employment of various treatment protocols and chest closure techniques. Regarding long-term outcome and quality of life, the chapter will contain comparative analysis of different treatment strategies from major published data focusing on closed chest irrigation, open packing and negative pressure wound therapy. Techniques of soft tissue closure and chest stabilization will be explored in detail for achievement of uncomplicated wound healing and improved quality of life. Management of complicated wound healing will also be discussed in which extensive residual defects with sternal bone loss require a complex approach including replacement of sternal bone mass with bone auto- or allograft and advancement of reliable musculocutaneous flap, with an overview of advancement flap advancement suitability for covering residual defects will be present as well as the concept of a multidisciplinary approach between cardiothoracic and plastic surgeons for chest wall repair.
