  • The aim of this work was to define the cut-off point for different methods of FDG-PET evaluation of temporal lobe epilepsy (TLE) and to define accuracy of these methods. 58 consecutive patients with proven hippocampal sclerosis (HS), and 27 oncologic patients without brain involvement (control group) were investigated by PET. ROC analysis was utilized for the assessment of accuracy of semi-quantitative parameters of manual evaluation by two operators as well as automated brain analysis by two software. Results support hypothesis, that FDG-PET is a reliable diagnostic tool for discrimination between patients with HS and control group. Sensitivity and specificity is at the level of 90% approximately, regardless the method of evaluation we used. Furthermore FDG-PET is capable to correctly predict the side of operation in 100% of patients with HS. Completely automated analysis is less laborious and operator’s independent. It gives the same results as careful manual assessment.

  • Cílem této práce bylo definovat hraniční hodnotu pro různé metody vyhodnocení FDG-PET u temporální epilepsie (TLE) a definovat přesnost těchto metod. 58 konsekutivních pacientů s prokázanou hippokampální sklerózou (HS) a 27 onkologických pacientů bez postižení mozku (kontrolní skupina) bylo vyšetřeno pomocí PET. ROC analýza byla použita pro vyhodnocení přesnosti semikvantitativních parametrů manuálního vyhodnocení 2 operátory a také automatického vyhodnocení 2 programy. Výsledky podporují hypotézu, že FDG-PET je spolehlivý diagnostický nástroj pro odlišení pacientů s HS od kontrolní skupiny. Senzitivita a specificita je kolem 90 % bez ohledu použitou metodu. Navíc FDG-PET je schopna správně predikovat stranu operace ve 100 % pacientů s HS. Zcela automatizovaná analýza je méně pracná a nezávislá na operátorovi. Dává stejné výsledky jako pečlivé manuální vyhodnocení. (cs)
