Description	Prospective study aimed at screening of polymorphisms and somatic mutations in blood and tissue samples of NSCLC patients. Emphasis on rational selection of therapy schemes and relationships between genetic predictors and the overall response to therapy. 150 patients with confirmed NSCLC stage III.A., III.B and IV. will be indicated to chemotherapy according to the status of genetic polymorphisms or targeted biological therapy based on occurrence of somatic mutations. Monitoring of efficiency of common therapy approaches in correlation with inherited genetic polymorphisms (ERCC1, ERCC2/XPD and XRCC1) and somatic mutations developed within the tumor tissue (EGFR1, k- ras). Diagnosis, dispensarization and follow up provided by Department of pulmonary disease, Faculty hospital Plzen. Molecular-biology examination and evaluation provided by Genomac laboratory. (en) Prospektivní studie zaměřená na sledování genových polymorfismů a detekci DNA variací ve tkáních nemalobuněčných plicních karcinomů. Orientace na volbu terapie a hledání vztahů mezi genetickými parametry a efektem chemoterapie a biologicky cílené léčby. 150ti nemocným s morfologicky ověřeným karcinomem stádií IIIA, IIIB a IV bude indikována chemoterapie podle výsledku vyšetření vybraných genových polymorfismů, nebo biologicky cílená léčba, budou-li prokázány mutace nebo amplifikace genu EGFR. Ve studii bude sledována efektivita současných standardních léčebných postupů ve vztahu ke genovým polymorfismům nemocných a ve vztahu k některým variacím vzniklým v nádorové tkáni (mutace a amplifikace EGFR, mutace k-ras). Diagnostiku, léčení a dispenzarizaci nemocných zajistí Klinika TRN v Plzni, molekulárně-biologické analýzy zajistí Laboratoř molekulární onkologie, Genomac International v Praze.
