  • We read with great interest the results of the prospective randomized EORTC 20981 phase III intergroup study.1 This report will have an extraordinary impact on decision making in clinical practice worldwide. Van Oers et al1 concluded that B-cell lymphoma 2 (BCL2)/immunoglobulin heavy chain (IgH) polymerase chain reaction (PCR) status at the end of induction treatment is not predictive for progression-free survival (PFS) in patients with relapsed or resistant follicular lymphoma. We fully agree with the statement that pretreatment levels of BCL2/IgH transcript correlate with tumor load and therefore with patient outcome. However, we are doubtful about the absence of a predictive value of BCL2/IgH negativity after induction. BCL2/IgH PCR status has been established as a reliable marker of residual disease, andmanytrials have confirmed the predictive value of molecular complete remission (CR) achievement on PFS.2-5 This discordance was influenced by many clinical factors.
