  • Background: Transcranial sonography (TCS) is able to image in B-mode infratentorial and supratentorial structures. It could be used for diagnosis and differential diagnosis of various intracranial pathologies. Methods: The authors review the contribution of TCS to diagnosis and different diagnosis of the Parkinson?s disease (PD). Results: TCS can evaluate changes of echogenicity and area of substantia nigra (SN) in Parkinson?s disease. Hyperechogenic enlarged SN can be detected in about 90% PD patients and also in about 10% healthy population as a marker of subclinical injury of the nigrostriatal system. Hypoechogenic ncl. raphe could be detected in PD patients with unipolar depression and this finding correlates also with incontinency. Evaluations of echogenicity of thalamus, ncl. lentiformis, ncl. caudatus, ncl. dentatus and cerebellum and measurement of enlargement of ventricular system could be used for differential diagnosis of movement disorders.
