| - Cíle: Stanovení volných plazmatických metanefrinů se jeví jako nejpřesnější metoda v biochemické diagnostice feochromocytomu, ale není známa jejich efektivita u neselektovaných pacientů s nízkou pretestovou pravděpodobností. Metodika: Při průřezové multicentrické studii zahrnující 1260 subjektů jsme hodnotili diagnostickou efektivitu plazmatického metanefrinu a normetanefrinu u pacientů s akcelerovanou hypertenzí, paroxysmální hypertenzí, výskytem %22flushe%22 a rovněž u menšího počtu pacientů s incidentalomem nadledviny a genetickou predispozicí pro feochromocytom. Výsledky: Feochromocytom byl diagnostikován a histologicky prokázán u 25 subjektů (2%), u ostatních 1235 nebyla dlouhodobým sledováním ani zobrazovacími metodami diagnóza prokázána. Kombinovaný test plazmatických metanefrinů se ukázal jako vysoce senzitivní (100%) a specifický (96,7%) test s negativní prediktivní hodnotou 100%. Závěr: Uspokojivá diagnostická efektivita volných plazmatických metanefrinů umožňuje jejich použití jak (cs)
- Objectives: Detection of free plasma metanephrines seems to be the most exact method for biochemical diagnosis of pheochromocytonoma, but their diagnostic efficacy in the common low-risk clinical settings is debated. Methods: A cross-sectional multicentre study including 1260 subjects assessed the diagnostic efficacy of free plasma metanephrine and normetanephrine in low-risk patients screened for resistant or markedly accelerated hypertension, paroxysmal hypertension, %22flushes%22, and, in a small proportion, for adrenal incidentaloma or genetic predisposition to pheochromocytoma. Results: Pheochromocytoma was identified and verified by histology in 25 subjects (2%), with the diagnosis not confirmed by long-ter, follow-up or use of imaging techniques in the remaining 1235 individuals. The combined assay of free plasma metanephrines was a highly sensitive (100%) and specific (96,7%) measure, yielding a negative predictive value of 100%.
