Description
| - Strategies to enhance motor system activation and motor plasticity after hemiparetic stroke include increased afferent input. We have previously shown that employment of affective prosody in voice instruction while stroke patients perform hand movements with the impaired hand leads to enhanced activation of motor cortices detected by functional MRI (fMRI). The present pilot study tested the hypothesis that this enhancement is associated with strengthening of effective connectivity among the area processing affective prosody (right posterior superior temporal sulcus, RpSTS), the supplementary and cingulate motor (SMA/CMA) cortices and the contralateral primary motor cortex (M1). - Strategie uplatňované pro zvýšení aktivace motorického systému a motorické plasticity po cévní mozkové příhodě (CMP) s reziduální hemiparézou zahrnují zvýšení aferentace. Dříve jsme použitím funkční MR (fMRI) ukázali, že použití afektivní prozodie v hlasových instrukcích, při nichž pacienti po CMP provádějí pohyby postiženou rukou vede k zvýšení aktivace motorických kortikálních oblastí. Tato pilotní studie testovala hypotézu, že toto zvýšení je spojeno s posílením efektivní konektivity mezi oblastí zpracovávající afektivní prosodii (pravý sulcus temporalis superior ve svém posteriorním průběhu, RpSTS), suplementární a cingulární motorickou kůrou (SMA/CMA) a kontralaterální primární motorickou kůrou (M1). Tři pacienti, praváci, s reziduální hemiparézou po cévní mozkové příhodě v povodí arteria cerebri media s ušetřením motorického kortexu a RpSTS byli studování užitím BOLD fMRI během provádění motorické úlohy postiženou rukou ve dvou různých modech vedení hlasem terapeuta.
