Description
| - Occlusion of the internal carotid artery (CAO) is associated with a high mortality rate and frequent disability in survivors. Even in patients with good clinical recovery there is a high risk of recurrent stroke, mainly in those with impaired cerebral vasomotor reactivity (CVR). Current evidence based therapeutic options for patients with symptomatic CAO include antithrombotic medication and control of vascular risk factors. For stenosis of the contralateral internal or ipsilateral external carotid artery, endarterectomy or percutaneous transluminal angioplasty may be considered. Ongoing symptoms may cease after tapering antihypertensive medications. Extracranial to intracranial (EC/IC) arterial bypass surgery has been used since 1967 in patients with CAO. However, the international randomized EC/IC Bypass Study (1985) failed to con.rm the e.ectiveness of EC/IC bypass for preventing cerebral ischemia in patiens with symptomatic CAO when compared to those assigned to the best medical care. - Okluze vnitřní karotické tepny (KAO) je spojena s vysokou mortalitou a u přeživších s vysokou frekvencí invalidity. Dokonce u pacientů s dobrou klinickou úzdravou je vysoké riziko recidivy iktu, především u těch s porušenou cerebrální vazomotorickou reaktivitou (CVR). Současné léčebné možnosti založené na důkazech zahrnují u pacientů se symptomatickou KAO antiagregační terapii a kontrolu cévních rizikových faktorů. Endarterektomie nebo perkutánní transluminální angioplastika mohou být zváženy v případě stenózy kontralaterální vnitřní nebo ipsilaterální zevní karotidy. Pokračující symptomy mohou ustoupit po snížení dávky antihypertenzní terapie. U pacientů s KAO je od roku 1967 užíváno chirurgické provedení extrakraniálně-intrakraniální (EC/IC) arteriální anastomózy. Mezinárodní randomizovaná studie EC/IC Bypass Study (1985) však u pacientů se symptomatickou KAO nepotvrdila účinnost EC/IC anastomózy při prevenci mozkové ischémie ve srovnání s nejlepší konzervativní terapií.
