  • We have found that the determination of thiodiglycolic acid (TDGA)in urine may help to characterize metabolic imbalance of substances participating in methionine synthesis, which leads to hyperhomocystinuria. From the metabolic scheme, based on proper combination of know facts, we attempted to theoretically explain and to demonstrate the possibilities of TDGA formation via different ways of homocysteine transformation. This scheme was used in evaluating results obtained from testing urine of a woman suffering from impaired function of methionine synthese reductase. Her disease was declared as CblE type of homocystinuria. The amount of TDGA excreted in her morning urine was very sensitive to changes in combination of N5-formyl tetrahydrofolate, betaine and vitamin B12 used in her treatment. Vitamin B12 given in the evening either alone or together with betaine increased the TDGA excretion in the morning urine up to ten times.

  • Bylo zjištěno, že stanovení kyseliny thiodiglykolové (TDGA) v moči může pomoci charakterizovat nerovnováhy látek účastnících se na syntéze methioninu, který vede k hyperhomocystinurii. V metabolickém schématu, založeném na vhodné kombinaci známých faktů, jsme se pokusili teoreticky objasnit a demonstrovat možnosti tvorby TDGA různými cestami při transformaci homocysteinu. Toto schéma bylo využito pro vyhodnocení výsledků obdržených při testování moči ženy trpící poruchou funkce methionin synthesy reduktasy. Její nemoc byla označena jako CblE typ homocystinuriu. Množství TDGA vyloučené v její ranní moči urine bylo velmi citlivé na změny kombinované podávaných N5-formyl tetrahydrofolátu, betainu a vitamin B12, které byly užity k její léčbě. Vitamin B12 podávaný večer samotný nebo v kombinaci s betainem zvýšil vylučování TDGA v ranní moči až desetinásobně. Na druhé straně, za nepřítomnosti vitaminu B12, betain v kombinaci s N5-formyl tetrahydrofolátem brání výskytu TDGA v ranní moči. (cs)
