  • Arteriovenous malformation (AVM) is a benign cerebrovascular disease presenting mainly with hemorrhage (50%), seizures (25%), headache (15%), or rarely by ischemia that is caused by steal phenomenon. Clinical presentation comes at a relatively young age where the majority of patients are in their 40s. According to Ondra, the annual rupture rate is between 2% and 4%. Poor outcome (Rankin score 3-5, permanent deficit) is reported in 20% to 30% of the cases. Therapy of AVMs is based on three therapeutic modalities: surgical resection, endovascular embolization and stereotactic radiosurgery (LGK or LINAC). The goal of the therapy is complete AVM occlusion. We present results of arteriovenous malformation (AVM) treatment performed at our institution during the period 1998–2006. From an original sample of 149 patients, 71 patients underwent surgery, 43 underwent endovascular embolization procedures and 16 were referred to radiosurgery unit. A watch and wait strategy was chosen for the remaining 27 patients.

  • Arteriovenózní malformace (AVM) je benigní cerebrovaskulární onemocnění, které se prezentuje nejčastěji krvácením (50%), záchvaty (25%), bolestmi hlavy (15%), nebo zřídka ischemií mozkové tkáně způsobenou steal fenoménem. AVM se prezentují nejčastěji v mladším dospělém věku, většina pacientů nedosáhne 40 let. Podel prací Ondry je roční pravděpodobnost ruptury mezi 2% a 4%. Neblahý výsledek, tj. Rankinovo skóre 3-5, krvácení z AVM je popsán u přibližně 20% až 30% pacientů. Terapie AVM je založena na třech pilířích: mikrochirurgické resekci, endovaskulární embolizaci a stereotaktické radiochirurgii. Jediným možným cílem při léčbě AVM je její kompletní vyřazení. V této práci představujeme výsledky léčby AVM provedené v letech 1998 až 2006 na naší klinice. Z původní sestavy 149 pacientů 71 podstoupilo resekci, 43 endovaskulární embolizaci, 16 bylo odesláno k ozáření na pracoviště Leksellova Gama nože a u 27 pacientů byla zvolena observace. (cs)
