  • A 30-year old female with persisting paresthesia of acral forelimbs, right hand weakness and double vision underwent brain angiography. This revealed both internal carotid artery stenoses and stenoses of the anterior cerebral arteries. After contrast medium application characteristic moyamoya markers (coiled and elongated vessels with „smoke puffs carried away by breeze) were found. The histological findings included obstruction of vessel lumen, fibrocellular intimal thickening, tortuosity and disruption of internal elastic lamina and, attenuation of media together with decreased SMA positivity. We confirmed a defect of the internal elastic membrane of the muscular arteria using elastica and alcian blue (Müller) stains and progressive intimal thickening accompanied by findings of abnormal smooth muscle cells and, VEGF and VEGFR expression in intima.
