  • Introduction: Endoscopic third ventriculostomy ( ETV) is considered a safe procedure and is method of choice in treatment of obstructive hydrocephalus nowadays. In children younger than 6 to 24 months respectively, however, some authors report lower efectiveness ranging between 0 – 64%. The reasons o ETV failure are discussed : hyporesorption in patients with obstruction as a consequence of haemorhage or infection, suboptimal ETV performance, especially in premature newborns or the theory of different cerebrospinal fluid circulation in newborn babies.Materials and methods: Between January 2005 and December 2006 in our clinic 14 patientns younger than 6 months presentig with obstructive hydrocephalus were treated endoscopically. Obstruction was revealed by preoperative MRI. The etiology of hydrocephalus was congenital aqueduct stenosis in 5 patients, posthemorrhagic obstruction in 8 patients and combination of posthemorhagic and postinfection etiology in 1 patient.   • Úvod: Endoskopická ventrikulo-cisternostomie (ETV) je dnes považována za bezpečnou proceduru a metodu první volby při léčbě obstrukčního hydrocefalu. U dětí mladších než 6-24 měsíců je však některými autory popisována nižší úspěšnost, pohybující se mezi 0-64%. Jako příčina nižší úspěšnosti je uváděno: hyporesorpce u pacientů s prokázanou obstrukcí v případě krvácení či infekce, obtížné provedení ETV zvláště u nezralých novorozenců a teorie odlišné cirkulace mozkomíšního moku u novorozenců. Materiál a metoda: V období mezi lednem 2005 a prosincem 2007 bylo na naší klinice endoskopicky operováno 14 dětí mladších 6 měsíců pro obstrukční hydrocefalus. Obstrukce byla prokázána předoperační MRI. U 5 pacientů byla příčinou hydrocefalu stenosa mokovodu, u 8 posthemoragický hydrocefalus a u jednoho pacienta pak kombinovaný posthemoragický a postinfekční hydrocefalus. EV byla hodnocena jako úspěšná u pacientů, kde nebylo nutné implantovat V-P drenáž. Výsledky: ETV byla úspěšná u 8 pacientů u (cs)
