| - Klinické rysy lumbální spinální stenózy (LSS) jsou heterogenní a u podstatné části nemocných nedochází k jejich deterioraci a způstávají nezměněné. Cílem studie bylo zhodnotit klinický rozvoj během 4-letého období sledování se zřetelem na typ léčby. Do sledování po dobu 53 +- 6 měsíců bylo zařazeno 39 pacientů s LSS s průměrným věkem 63,5 let (rozmezí 42-82 let). Mezi sledovanými parametry byla přítomnost a intenzita klidové bolesti, přítomnost neurogenních klaudikací (NK), přítomnost paréz, neurologické skóre a Oswestry skóre neschopnosti. Pacienti byli léčeni konzervativně, chirurgická léčba byla indikovaná v případě jejího selhání. Na začátku studie mělo 74 % pacientů NK, 79 % klidové bolesti, intenzita bolesti dle vizuální analogové škály byla 6,3 + 1,8, parézy dolních končetin byly přítomny u 26 %, Oswestry index 49,7 + 17,8 %. Chirurgická léčba byla indikována u 12 pacientů (31 %). Na konci sledovaného období byla většina parametrů nezměněna a nebyla zjištěna statisticky významná změna jak u kon (cs)
- The clinical features of lumbar spinal stenosis (LSS) are heterogeneous and a substantial proportion of patients do not automatically deteriorate and remain unchanged. The aim of this study was to evaluate a clinical development during 4 four year follow-up with respect of a treatment modality. Thirty-nine patients with LSS participated in the study, mean age was 63.5 years (range 42-82 years). The follow-up was 53 + 6 months. We evaluated following parameters: the presence and the intensity of rest pain, the presence of neurogenic claudication (NC)), the presence of weakness, the score of neurological examination, and disability (Oswestry disability index (ODI)). Patients were managed conservatively, surgical decompression was performed in case of conservative treatment failure.
