Description	Hallux vagus is the most frequent deformity of big toe. Multiple surgical procedures were described, quite often with tiny differences. Discussion deals with the issues of the first metatarsal instability in patients with rheumatoid arthritis. Conclusion: Most patients with rheumatoid arthritis suffer from polyarticular affliction. It is necessary to assess the extent of joint deformation and choose an individual approach. Intention of the surgery is to achieve the plantigrade position of the foot. Lapidus surgery, when done properly, will allow, through the correction of the first MTT varus deformity to remedy the valgus position of the big toe and ensure sufficient stability of the first ray during walking. (en) Hallux valgus je nejčastější deformita palce. Bylo popsáno značné množství operačních metod, které se liší mnohdy v maličkostech. V diskuzi je uváděna problematika nestability I. metatarzu u pacientů s revmatoidní artritidou. Závěr: Většina pacientů s revmatoidní artritidou má polyartikulární postižení. Je třeba zhodnotit míru kloubního postižení a zvolit individuální přístup. Cílem operací na noze je dosáhnout plantigrádního postavení nohy. Operace podle Lapiduse při správném provedení umožní korekcí varózní deformity I. MTT napravit valgózní postavení palce a zajistit dostatečnou stabilitu prvního paprsku při chůzi.
