Ontheotherhand,wecanseethetrendtowardearly surgeryin asymptomaticpatients withseverevalvulardisease.The operationbeforeventriculardilatationanddysfunctionimproves earlymortality,aswellaslong-termpostoperativesurvival.It wasthedevelopmentofechocardiographywithdetailednon- invasiveevaluationofthevalvularpathologyanditsconse- quences,whichenabledthequickprogressinvalve-sparing surgery.Theabsenceoflife-longanticoagulationandlowsurgi- cal mortalityinasymptomaticpatientswithgoodventricular functionfacilitatestheearlyindicationforsurgery. (en) Euro Hert Survey objevila mnoho pacientů s těžkými symptomatickými chlopenními vadami, kteří nebyli indikováni ke kardiochirurgickému výkonu. Důvodem byl vysoký věk a dysfunkce levé komory. Nová Evropská guidelines specifikují na jedné straně možnost operací vysoce rizikových pacientů ve specializovaných centrech. Na druhé straně vidíme trend k včasným operacím asymptomatických pacientů s těžkou chlopenní vadou. Euro Hert Survey objevila mnoho pacientů s těžkými symptomatickými chlopenními vadami, kteří nebyli indikováni ke kardiochirurgickému výkonu. Důvodem byl vysoký věk a dysfunkce levé komory. Nová Evropská guidelines specifikují na jedné straně možnost operací vysoce rizikových pacientů ve specializovaných centrech. Na druhé straně vidíme trend k včasným operacím asymptomatických pacientů s těžkou chlopenní vadou. (cs)
