| - 2. Cílem studie je vyhodnocení prediktivní hodnoty metody PET po 4 cyklech chemoterapie BEACOOP u pacientů s pokročilým stadiem Hodgkinského lymfomu zařazených do studie HD 15 studijní skupiny GHSG. 14 pacientů z 50 léčených ve studii HD 15 mělo pozitivní PET vyšetření po 4 cyklech chemoterapie. 13 z nich mělo iniciálně velkou masu lymfomu v mediastinu a dva z nich byli léčeni salvage terapií a následnou transplantací hematopoetických kmenových buněk , neboť tumor vykazoval neadekvátní odpověď. Ve skupině s pozitivním PET vyšetřením po 4 cyklech chemoterapie měla většina pacientů iniciálně velkou masu v mediastinu. Tento výsledek podporuje teorii, že velká mediastinální masa je rizikovým faktorem a vyžaduje intentivní léčbu. Nicméně, vzhledem k dosud nízkému počtu pacientů a krátké době sledování , nebyla dosud stanovena výpovědní hodnota PET jako prognostického faktoru u těchto nálezů. (cs)
- 14 out of 50 pts treated within HD 15 study GHSG had positive PET scan after 4 cycles of the chemoterapy. 13 of these had initially a large mediastinal mass and two of them were trated by salvage therapy with high-dose therapy with autologous stem – cell support, because of tumor inadequate response. In our analysis we have found that PET4 positive patiens belong mostly to the group with initial large mediastinal mass. This fading supports again well known observation that the large mediastinal mas sis the high rist facto rand requires intensive type of treatment. However, considering still low number of patiens and short follow-up, any definite conclusion regarding the prognostic impact of this finding is pending.
