Description	Recent studies have proposed that in the pathophysiology of progression of congestive heart failure combined with chronic kidney disease rather the interaction of renin-angiotensin-aldosterone system (RAAS) with other vasoactive systems has more important role than the activity of RAAS itself. The aim of the present study is to evaluate the role of RAAS and cytochrome P-450 (CYP)-derived metabolites in the pathophysiology of congestive heart failure combined with the chronic kidney disease. Experiments should also elucidate whether combined pharmacological interferences into CYP-metabolic pathway and RAAS will exhibit additional protective effects on the progression of congestive heart failure and chronic kidney disease when compared with isolated pharmacological blockade of the RAAS. (en) Nejnovější studie prokazují, že v patofyziologii progrese chronického srdečního selhání kombinovaného s chronickým onemocněním ledvin má důležitější úlohu interakce renin-angiotenzin-aldosteronového systému (RAAS) s ostatními vazoaktivními systémy než samotná aktivita RAAS. Tato studie by měla objasnit úlohu interakce RAAS s metabolity kyseliny arachidonové, které vznikají cestou cytochromu P-450, v patofyziologii chronického srdečního selhání v kombinaci s chronickým onemocněním ledvin. Na základě našich pilotních studií chceme rovněž ověřit hypotézu, že selektivní farmakologické zásahy do metabolické cesty cytochromu P-450 jsou schopné zmírnit rozvoj chronického srdečního selhání a progresi chronického onemocnění ledvin. Experimenty by měly rovněž objasnit, zda kombinované farmakologické zásahy do metabolické cesty cytochromu P-450 a RAAS vykazují aditivní nefro- a kardioprotektivní účinky v porovnání s izolovanou farmakologickou blokádou RAAS.
