  • The individual vote of step by step therapy of deep infection of joint substitution on ability of a bacterial agens to form a biofilm. In the case that agens has not the ability to form a biofilm antibiotics can easily reach the upper part of an implant.One-reimplantation has in common worth results with treatment of infection but more better functional long term results than the two-stage reimplantation. On case when agent is not biofilm producer we supposed better results with one-stage procedure.The individual approach is necessary in indicated cases get clearly simpler and for an patient less sophisticated manner of treatment an optimal result with treatment of infection joint replacement.The developing of biofilm is a main problem in treatment of endoprothesis infection.Detection of biofilm producers is important before strating treatment.   • Individuální volba postupu léčby hluboké infekce kloubní náhrady na základě schopnosti bakteriálního agens formovat biofilm. V případě, že agens nemá schopnost formovat biofilm angtibiotika mohou snáze dosáhnout povrchu implantátu. Jednodobá reimplantace má obecně horší výsledky při sanaci infekce, ale výrazně lepší funkční, dlouhodobé výsledky než dvoudobá reimplantace.Rychle stanovení agens za pomoci multiplexní a kvantitativní PCR umožní racionální terapii jak chirurgickou tak i antimikrobiální.Biofilm zajišťuje ochrannou bariéru patogena proti účinkům imunitního systému a antibiotik.
