  • Finding of clonal chromosomal aberrations in plasma cells is considered as one of the most important and independent prognostic factor in patients with multiple myeloma (MM). The aim of the study was to assess the frequency and prognostic value of the most common chromosomal aberrations in a homogenous series of patients with newly diagnosed multiple myeloma enrolled to the phase III trial CMG 2002. In this study 208 patients enrolled until December 31, 2005 were included. All patients underwent single autologous bone marrow transplantation according to protocol of the trial after 4xVAD induction chemotherapy and were randomized after transplantation to two maintanance/consolidation treatment arms. All patients gave written informed consent to the proposed therapeutic procedure and to the provision of samples for research purposes.   • Za významný prognostický faktor je u nemocných s mnohočetným myelomem (MM) považován nález klonálních chromozomových aberací v plazmatických buňkách nález klonálních chromozomových aberací v plazmatických buňkách v době stanovení diagnózy. K nejčastějším nálezům aptří delece RB1 genu v oblasti 13q14 a/nebo ztráta celého chromozomu 13 (střední až špatná prognóza) a translogace IgH genu v oblasti 14q32 (většinou špatná prognóza). K nepříznivým prognsotickým faktorům patří i amplifikace oblasti 1q21 a delece tumor supresorového genu p53. cílem proejktu bylo zjištění frekvence a prognostického významu specifických chromozomových změn u nemocných s MM zařazených v letech 2003-2006 do klinické studie CMG 2002. Konvenční cytogenetickou analýzou a analýzou imunnofluorescenčně znamčených plazmocytů metodou cIg-FISH jsme vyšetřili 208 nově diagnostikovaných pacientů s MM. U 52,9 % nemocných jsme detekovali aberace chromozomu 13, přestavby IgH genu jsme prokázali u 58,6 % pacientů. Deleci genů p53 a zisk chromoz (cs)
