Description	Proteinuria together with arterial hypertension is a common complication in children after renal transplantation. It reduces survival of the renal graft as well as survival of transplanted patients. Angiotensin-converting enzyme inhibitors reduce proteinuria in adult as well as in paediatric patients with proteinuric nephropathies. The efficacy on proteinuria in patients after renal transplantation has been proved in adults but never in children. The aim of the proposed project is to investigate the efficacy of ACE-inhibitors to reduce proteinuria in children after renal transplantation. We hypothesized reduction of proteinuria could stabilize of even improve graft function. The proposers are members of a paediatric renal transplantation team at the Univ.Hospital Motol. The concentration of patients from the whole Czech Republic in a single centre gives an unique presumption to study a large group of transplanted children. (en) Proteinurie je společně s hypertenzí častou komplikací u dětí po transplantaci ledviny. Proteinurie stejně jako hypertenze zkracuje u dospělých pacientů přežívání transplantovaných ledvin i transplantovaných pacientů. Inhibitory angiotensin-konvertujícího enzymu snižují proteinurii u chronických onemocnění nativních ledvin u dospělých i dětských pacientů. Účinnost léčby proteinurie po transplantaci ledvin pomocí ACE-inhibitorů byla zatím prokázána jen u dospělých. Cílem navrhovaného projektu je zjistit v prospektivní intervenční studii, zda ACE-inhibitor sníží proteinurii a zlepší účinnost antihypertenzní léčby i u dětských pacientů po transplantaci ledviny. Většina navrhovatelů a spolupracovníků pracuje v transplantačním centrum FN v Motole, které jako jediné pracoviště provádí transplantace ledviny u dětí z celé ČR, čímž jsou zajištěny personální předpoklady pro úspěšné řešení navrhovaného projektu.
