| - Projekt si klade za cíl popsat na dostatečně velkém souboru klinický korelát clopidogrelové rezistence pomocí testování celkem 500 pacientů na účinek léku přístrojem VerifyNow a jejich klinickým sledováním. Dalším cílem je definovat genetické determinanty, které jsou příčinou clopidogrelové rezistence s nalazením nejčastějších mutací a definovat mutace, které jsou (činaopak nejsou) ovlivnitené zvýšením dávky clopidogrelu.. V projetku se budeme snažit otestovat hypotézu, zda rezistence na clopidogrel je u alespoń části pacientů ovlivnitelná podáváním vyšších dávek tohoto léku (místo nyní standardních 75mg/den 150mg až 225mg/den) či záměnou za lék jiný. Očekáváme, že práce umožní lépe identifikovat pacienty ohrožené nedostatečným efektem tohoto léku genetickým vyšetřením a u části z nich nabídne dosažení dostatečného efektu léku zvýšením dávky či záměnou za lék jiný. - The aim of the project is to describe on the sufficient number of patients the clinical impact of resistance to ADP antagonists by testing of 500 patients with the machine VerifyNow and by clinical follow-up of the patients. We will test the hypothesis, whether the resistance to clopidogrel could be influenced by higher doses of this drug (instead of 75mg/day, which is currently used, either 150mg or 225mg/day) or by replacement with ticlopidin Genetic determinants associated with resistance to ADP antagonists will be determined. The aim of genetic analysis is to find the mutations, which are (or which are not) influenced by higher dose of clipidogrel. We anticipate, our work enables better definition of the patients, who are at risk from insufficient effectiveness of ADP antagonists, and, at least in some of them, the effective treatment will be achieve.
